Evaluate and treat patients with physical and functional
limitations, developing and implementing individualised treatment
plans. Collaborate with the interdisciplinary rehab team,
physicians, and nursing staff to support patient recovery goals.
Document evaluations, progress notes, and discharge summaries in
compliance with facility and regulatory standards. Provide staff
education and mentorship to PTAs and rehab aides as needed.
Requirements Active, unencumbered Physical Therapist licence in the
applicable state. Minimum 1 year SNF or post-acute rehab experience
preferred. Current BLS/CPR certification. Strong documentation
