Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days, nights - Employment Type: Travel Job Title: Physical Therapist (PT) – Home Health (Field Role) Shift: Day 5x8 Hours (08:00 AM – 05:00 PM) Schedule Options: Tuesday–Saturday or Wednesday–Sunday Territory: Berkeley / Oakland, CA Setting: Home Health (Field-Based) Job Type: Contract / Travel-Friendly Mileage: IRS Standard Reimbursement Job Summary We are seeking a Home Health Physical Therapist (PT) for a field-based position covering the Berkeley/Oakland territory. The PT will provide in-home skilled physical therapy services, focusing on patient mobility, rehabilitation, and functional improvement. This role offers flexibility, autonomy, and guaranteed hours based on productivity. Key Responsibilities - Conduct in-home physical therapy evaluations and treatments - Develop individualized plans of care - Complete documentation using OASIS - Educate patients and families on exercises and safety - Coordinate care with interdisciplinary teams - Maintain productivity of 25 visits per week - Travel between patient homes within assigned territory Required Experience & Skills - OASIS experience required - Home Health experience preferred (will consider no HH experience) - Strong clinical assessment and documentation skills - Ability to work independently in a field-based role Licensure & Compliance - California PT license preferred - Pending CA license accepted (must have applied at submission) - COVID exemptions permitted Productivity & Pay Structure - 25 visits = 40 guaranteed hours - Guaranteed hours only if productivity is met - Rate may vary based on territory and experience Compensation & Benefits - Mileage reimbursed at IRS standard rate - Weekly guaranteed hours (based on visit completion) About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
