The ideal candidate must have a strong background in home health care, demonstrating proficiency in assessing, planning, and implementing effective physical therapy interventions for patients in the comfort of their homes. As a per diem Physical Therapist, you will contribute to the overall well-being of our patients by promoting mobility, independence, and optimal functioning. Responsibilities: Conduct thorough assessments of patients' physical conditions, identifying areas of impairment, functional limitations, and setting realistic goals for rehabilitation. Develop and implement individualized treatment plans based on assessment findings, incorporating evidence-based practices to maximize patient outcomes. Provide skilled physical therapy services to patients in accordance with physician orders, addressing issues such as mobility, strength, balance, and pain management. Collaborate with interdisciplinary team members, including nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care. Educate patients and their families on therapeutic exercises, safety measures, and home exercise programs to enhance treatment outcomes. Document patient progress, treatment interventions, and any changes in the patient's condition accurately and timely in accordance with regulatory standards. Maintain open communication with the clinical supervisor, attending regular meetings and participating in case conferences to discuss patient care plans. Stay current with advancements in physical therapy practices and home health regulations, incorporating new knowledge into clinical practice. Adhere to ethical and professional standards, ensuring patient confidentiality and promoting a positive and compassionate care environment. Qualifications: Valid NV state license as a Physical Therapist. Minimum of 1 years of experience in home health physical therapy and/or in an acute care setting. Strong clinical skills with a focus on patient-centered care. Excellent communication and interpersonal skills. Ability to work independently and collaboratively within a team. Understanding of Medicare home health regulations and documentation requirements. Current CPR certification. Reliable transportation for home visits.
