Job Overview: In this role, you will deliver daily patient care in
a home environment. Your responsibilities will encompass a variety
of tasks, including but not limited to: managing cases and
providing comprehensive care for multiple patients with varying
levels of health needs, facilitating admissions to home care
services, conducting post-hospital assessments, handling
recertifications and discharges, educating patients, performing
dressing changes, administering IV therapy, caring for access
sites, and executing general skilled nursing in accordance with
established policies and procedures. Additionally, you will assist
with other home needs to ensure safety and appropriate care, such
as connecting patients with lifeline services, meals on wheels, and
information on public program availability. Timely and accurate
documentation will be maintained using the Epic computer system.
This is a Per Diem position, offering visits as needed while
adhering to all departmental guidelines. You will carry out your
duties in alignment with our mission, values, and service

Areas: Covering Franklin, Gasconade, Crawford & Warren Counties

year in a related field. Education: Graduation from an accredited
nursing program is required. Licensure: Must hold a current RN
license in Missouri and possess a valid driver's license.
Certifications: BLS CPR Certification from the American Heart
Association is mandatory. Other: The home health nurse will manage
a caseload of patients with diverse health conditions, providing
essential education, treatment, and emotional support to aid
recovery from injuries, illnesses, or chronic disease management.
Strong assessment skills and technical proficiency are required.
The ability to anticipate and respond to changes in patients'
clinical status is essential. Excellent communication and rapport
with patients and their families are crucial. The home health nurse
will also communicate effectively with physicians and other
healthcare professionals regarding patient care, ensuring concise
and accurate documentation of any changes in medication and care
